Same Day Appointments

We maintain some open appointment times for emergencies Monday – Friday. Feel free to call our office starting at 7:45 am on the day you wish to be seen. These spaces are reserved for “same day call ins”. We do not book these appointment times in advance. The earlier you call, the better the chance that we can accommodate your needs.

If you are a “walk in”, we will do our best to fit you in…no guarantees! Dr. Lieberman prides himself on seeing his scheduled patients on time.

We all know what it is like to have a sinus infection or an earache, so we will try to accommodate you.


Appointment Cancellations

We are privileged to provide medical and surgical treatment for our patients. We work diligently to maintain our high level of personalized service and strive to accommodate our patients’ needs for office visits. If you are not able to keep an appointment, please provide us with at least 24 hours notice. This courtesy on your part makes it possible to give your appointment to another patient.

We understand that emergencies arise occasionally with our patients, just as they do for us. When a patient fails to keep an appointment or give adequate cancellation notice, we cannot use that time to take care of others.


New Patient Forms & Information

Welcome to our office! Dr. Lieberman and his staff members strive to provide you with caring and comprehensive medical evaluations and the most advanced treatment options available.

Please bring a photo ID (e.g. drivers license), your insurance cards as well as a list of ALL of your current medications to your first appointment.

Also, the 3 following forms need to be downloaded and completed prior to your appointment. You can email your forms to [email protected] or print them at home and bring them to your appointment. If you prefer to fill them out in our office, please arrive 15 minutes prior to your scheduled time to do so.

Insurance and Payments

For those patients who do not have health insurance, payment is appreciated at the time of service. For those who do have insurance, all co-payments and any unmet deductibles need to paid for at the time care is rendered. Insurance plans vary and can be difficult to understand, but we will help you to determine what your out of pocket responsibilities are.

Depending on the insurance plan, there are some office based Ear, Nose and Throat services and Facial Plastic Surgery procedures that are subject to different co-pay and deductible rules than a straight office visit. We can help you to comprehend what your benefits are. Here are a few examples:

  • Control of a nose bleed
  • Nasopharyngoscopy
  • Cerumen (ear wax) removal
  • Checking the pressure in your ears (Tympanometry)
  • Removal of a foreign body from the nose or ears
  • Hearing tests (Audiometry)
  • Skin biopsies of moles or warts
  • Excisions of basal or squamous cell carcinomas (skin cancers)

In addition to cash and checks, you may pay Dr. Lieberman with Visa, MasterCard, Discover or American Express.


Prescriptions

Refill Policy

If you a need a prescription refilled, we suggest that you call your pharmacy first. Many time patients are not aware of additional refill opportunities.

Dr. Lieberman reviews all refill requests. As long as you contact us before 3:00 pm Monday – Thursday and before 12:00 pm on Friday, we will really try to handle your request the same day. Refill requests will only be considered for a condition that has been treated by Dr. Lieberman within the last 12 months and for those patients who have been compliant with Dr. Lieberman’s treatment plan (e.g. taking medications as suggested, keeping follow up appointments, etc.). Antibiotics and controlled substances (e.g. narcotics) will not be refilled automatically.

Please note that we are not allowed to e-scribe or call in controlled substance prescriptions. You will have to pick up a written prescription from our office.

Prior Authorizations for Prescriptions

If a prescription needs a prior authorization from your insurance company, please call them and request that they fax the necessary paperwork for that specific prescription to us at 772.569.9252. We will complete our end of the process as quickly as we can, but please keep in mind that we have no control over how long it will take your insurance company to respond.
